Investigators say a driver has been cited in a crash involving a car and a bicyclist that left a 64-year-old woman, who was a longtime nurse, dead in Green Oaks last summer.

The incident happened around 11:30 a.m. July 10 near St. Mary’s Road and Terre Drive in Green Oaks.

The Lake County Sheriff’s Office received a 911 call for a vehicle versus bicycle crash with injuries.

Deputies arrived and found a 64-year-old Green Oaks woman with significant injuries, according to Lake County Sheriff’s Office Deputy Chief Christopher Covelli.

Lake County Coroner Jennifer Banek said the woman was identified as Nancy Nozicka.

Daniel E. Quane, who was 81 and resided in Libertyville at the time, was driving a 2005 Toyota 4Runner southbound on St. Marys Road approaching Terre Drive.

Investigators initially thought Nozicka, who was on her bicycle, attempted to cross St. Marys Road from the west side of Terre Drive.

Quane struck the woman, leaving her critically injured.

Paramedics transported Nozicka to Advocate Condell Medical Center in Libertyville where she was later pronounced dead. Quane was not injured.

Covelli said in an update Saturday that an extensive review of the evidence was conducted and Quane was determined to be at fault.

An application on Nozicka’s phone played “an important role” in assisting investigators to determine the woman was not actually crossing the roadway at the time.

Investigators did not find that intoxicating substances or electronics contributed to Quane hitting the woman. “The crash appears to be a horrifically tragic accident,” Covelli said.

Quane was issued a citation for failure to reduce speed to avoid a traffic crash following the investigation.

According to her obituary, Nozicka leaves behind her husband and four children.

She led a 38-year career as a nurse at Northwestern Lake Forest Hospital and later became the hospital’s first heart failure coordinator, the obituary said.
