The McHenry County Coroner’s Office has released the identity of a man killed in a car crash running from Woodstock police early Friday morning.

An autopsy for Fabian Velazquez, 25, of Woodstock, is due to take place May 28, coroner officials said.

Velazquez died at Northwestern Hospital Huntley shortly after the violent one-vehicle crash on routes 47 and 176 in Lakewood, officials said.

Woodstock Police were called to the 200 Block of Douglas Street in Woodstock at 12:39 a.m. Friday for a domestic incident, authorities said.

Authorities said Velazquez discharged a gun in the residence while multiple innocent family members were inside.

No one was injured in the shooting, authorities said.

Velazquez left the scene in a vehicle before police arrived, authorities said, adding Velazquez was the sole occupant of the car.

Officers heading to the scene spotted Velazquez driving and attempted to perform a traffic stop, authorities said.

Velazquez disregarded police commands and drove southbound on Route 47 at a high-rate of speed, authorities said.

Authorities said Velazquez lost control of the vehicle near Route 176  in Lakewood after the vehicle veered to the right, went into a grassy area, went airborne and crashed onto Swanson Road, authorities said.

The coroners office said in their release Friday evening the vehicle rolled over during the crash.

The McHenry County Sheriff’s Office, Lakewood PD, and the Crystal Lake Fire Department arrived at the scene to extricate the driver and attempt life-saving measures.

The driver was transported to Huntley Northwestern Hospital where he later died, Lakewood police said.

A firearm was recovered at the scene matched the description of the weapon used in the residence on Douglas Street, police said.

The Woodstock Police requested the assistance of the McHenry County Major Investigations Assistance Team to assist in the investigation, officials said.
