The coroner has released the identity of a 43-year-old man who died after he was struck by an SUV while crossing the street in Waukegan early Saturday morning.

An autopsy performed Tuesday showed Juan Mentada, 43, of Waukegan, died as a result of blunt force injuries from a vehicle collision, according to Lake County Coroner Jennifer Banek.

The Waukegan Fire Department and Waukegan Police Department responded around 12:30 a.m. Saturday to the area of Grand Avenue and Berwick Boulevard for a vehicle crash with injuries.

Waukegan Deputy Police Chief Scott Chastain said a pedestrian, later identified as Mentada, was attempting to cross through traffic.

Mentada was nearly hit by multiple vehicles prior to a 2018 Nissan Rogue striking him.

The Nissan, which had been traveling in the eastbound lanes of Grand Avenue, stopped at the scene, Chastain said.

Mentada was transported to Vista Medical Center East in Waukegan where he was pronounced dead.

Police initially said Mentada was a Park City resident but the coroner said he was a Waukegan resident.

Chastain said the driver, a 20-year-old Park City man, remained at the scene and fully cooperated with investigators.

The driver did not appear to be impaired but provided toxicology samples, Chastain said.

The crash remains under investigation by the Waukegan Police Department Major Crash Unit.
