The incident was upgraded to a box alarm through the Mutual Aid Box Alarm System (MABAS) for numerous fire departments and ambulances to respond to the scene due to the number of people injured.

An investigation showed that a 2014 Honda CR-V, driven by Eric Fiador, 37, of Lindenhurst, was traveling southbound on Fairfield Road.

The Lake County Major Crash Assistance Team (MCAT) was called to the scene to conduct a crash investigation and reconstruction.

Fairfield Road was completely shut down for approximately four hours on the day of the crash.

Court documents said Fiador was acting in a reckless manner when he operated his vehicle while traveling at speeds in excess of the speed and left his lane of travel, causing the head-on crash.

Fiador survived the incident and was later released from the hospital.

Court records show the Lake County State’s Attorney’s Office approved charges in February against Fiador for one count of reckless homicide, a Class 3 felony.

A Lake County judge signed an arrest warrant for Fiador and he was processed on the warrant at the Lake County Jail in mid-April.

The Lake County State’s Attorney’s Office did not file a petition to detain Fiador pending trial and he was released following a First Appearance Court hearing.
